Court Ordered Probation Conditions

Body

Individuals sentenced to active supervision with the Probation Services department may be ordered to complete several conditions by the Court, or by their Probation Services Case Manager as approved by the Court. If you have questions about what you were ordered to complete on Probation, review your Judgment and Sentencing paperwork, the Order Establishing Conditions of Probation, and/or contact your Probation Services Case Manager.

Proof of Completion

Please submit proof of completed probation conditions in any of the following ways:

  • Forward or submit email attachments to benchprobation@clark.wa.gov
  • Directly e-mail the assigned Probation Services Case Manager
  • Fax paperwork to (360) 759–6881
  • Drop off completions in person at Probation Services

It is strongly recommended to contact our department or the assigned Case Manager to confirm we received proof of submissions.

To ensure your case manager can access the information from your treatment provider, please complete a Release of Information. You can fill out and sign the Release of Information form online through the link below.

Release of Information

Reporting to Assigned Case Manager

Some individuals on active supervision may be required to routinely report in person to their assigned Probation Services Case Manager. Report days are typically established during the initial meeting and intake, and by directive of the Case Manager. Clients with questions about their reporting schedule should directly contact their Case Manager via phone or e–mail.

Evaluation and Comply

The Court may order an individual to complete a specified evaluation or assessment from a state–certified treatment agency and comply with the evaluation’s recommendations for classes or treatment programs (if applicable). Upon completion, a copy of the evaluation with the recommendation must be submitted to Probation Services. If the recommendation is for a treatment program, a release of information for the assigned Case Manager must be signed so the treatment agency can send monthly compliance reports until successful discharge. If the recommendation is for a class, a completion certificate or letter of completion must be submitted.

Please note that some evaluation appointments may require individuals to bring additional paperwork, such as their Judgment & Sentencing, a copy of the police report, and/or their driver record.

  • Chemical dependency / substance use
  • Mental health
  • Domestic violence
