Funeral Homes

Before ordering certificates, funeral homes will need to register for an online profile. Once that has been completed, all your orders will be submitted online. Clark County Vital Records does not accept email or paper applications from funeral homes.

Profile registration:

  • Funeral home profile registration
    • Please email us to request an online account and link to your facilities.
       
  • Ordering certificates online
    • From the portal home page, enter your email. Users will receive a token/passcode in their email each time they log in. This will be in lieu of a password.
    • To place an order:
      • Go to Order Processing
      • Start a Fast Order
      • Enter Applicant Information
      • Choose shipping method
      • Enter Order Event Requests. When ordering a VA certificate, be ready to upload a copy of the DD214 or equivalent.
      • Enter your payment method.
      • Submit and validate the order.
         
  • Ordering correction copies
    • Mail or email the Affidavit for Correction to the state:
    • Send the original certificates that need replaced to Clark County Vital Records office.
      • Mail – this may delay the order.

      • Bring into the office:
        • 1601 E. Fourth Plain Blvd. Building 17, 3rd floor, Suite A327
    • Once they are received, Clark County Vital Records staff will call the Funeral Home to confirm we have received them, and they can place an order for the correction copies.
    • Place order for corrected copies online.
