Self-help form kits

The library sells self-help kits with forms and instructions on a variety of topics. Kits can be purchased with cash or credit/debit cards (fee applies). All prices include sales tax. Kits are not available for download due to copyright restrictions.

Purchases are final and there are no refunds. Librarians are not responsible for deciding if a kit is the correct one for your situation.

All kits are being mailed while the library is physically closed. Kits are mailed out once per week, so kits ordered during the week will be mailed out the following week. A $10 shipping and handling fee (S/H) is added to the price of each kit.

Contractor's Bond Claim - $25 ($15 kit + $10 S/H)

The kit includes forms and instructions to start a case against a contractor's bond by a homeowner or supplier/subcontractor when the contract was breached or performance was negligent. It does not cover procedures after the initial filing, and is not intended for electrical contractors.

Eviction by Landlord 20-Day Notice to Terminate (Residential) - $70 ($60 kit + $10 S/H)

The kit includes forms and instructions to process a basic eviction in a month-to-month tenancy or when the lease has expired and the tenant won't leave. It has information specific to Clark County but can be used in other Washington counties if you check for local ordinances or procedures that are different. It can be used with other notices (except the 14-Day Notice to Pay Rent or Vacate) if you change the pleadings to match the different notice and your situation.

COVID-19 note: The governor has issued a temporary moratorium on some evictions. That moratorium was extended through the end of June. The Northwest Justice Project has information about eviction during COVID-19, and an Eviction Help App. They also put out a webinar in August answering questions about the eviction moratorium. There is currently a temporary federal eviction moratorium. The Superior Court has entered a General Order about the Eviction Resolution Program that impacts the eviction process. Be aware that the situation is fluid and that you should verify there is no additional guidance issued. Eviction by Landlord 14-day Notice to Pay Rent or Vacate (Residential) - $60

(An attorney is updating the kit so it is currently not for sale. Prior kits are no longer valid.)
The kit includes forms and instructions to process a basic eviction when the tenant has stopped paying rent or the lease has expired and the tenant won't leave. It was completely rewritten in 2019 due to major changes in the law and is being updated now due to changes made in the 2020 legislative session.

The kit includes forms and instructions to request the restoration of gun rights lost due to Washington State convictions.

The kit includes forms and instructions to open and close a simple solvent estate in Washington when the person who died had a Will. It does not cover administration of the estate, or situations where an heir is a minor or incapacitated person.

Sealed Name Change for Domestic Violence Survivors - free download

The kit is available for download due to the generosity of the King County Law Library. You can change the heading on the forms to indicate Clark County rather than King County.

Small Estate Affidavit - $25 ($15 kit + $10 S/H)

The kit includes forms and instructions to transfer personal property of deceased Washington residents without probate for a small estate. It does not apply to estates where the debts have not been paid or provided for, where there is real estate such as a house or land, or to estates worth more than $100,000.

Starting a Civil Lawsuit in Superior Court - $25 ($15 kit + $10 S/H)

The kit includes forms and instructions to start a civil lawsuit in Superior Court against a person. It does not cover procedures after the initial filing and is not intended for District Court.

Note: The updated version corrected an error in the statute of limitations chart. Injury to person should be 3 years rather than 2 years under RCW 4.16.080.
