Self-help form kits

Body

The library sells self-help kits with forms and instructions on a variety of topics. Kits can be purchased with cash or credit/debit cards (fee applies). All prices include sales tax. Kits are not available for download due to copyright restrictions.

Purchases are final and there are no refunds. Librarians are not responsible for deciding if a kit is the correct one for your situation.

Self-help form kits can be mailed for an additional fee.

1. Eviction by Landlord 20-Day Notice to Terminate (Residential) - $60 (updated 10/2020)
The kit includes instructions and forms needed to process a basic eviction in a month-to-month tenancy or when the lease has expired and the tenant won't leave. It has information specific to Clark County but can be used in other Washington counties if you check for local ordinances or procedures that are different. It can be used with other notices (except the 14-Day Notice to Pay Rent or Vacate) if you change the pleadings to match the different notice and your situation.

COVID-19 note: The governor has issued a temporary moratorium on some evictions. Northwest Justice Project also has information about eviction during COVID-19 and they put out a webinar in August answering questions about the eviction moratorium. There is also a federal moratorium. Be aware that the situation is fluid and that you should verify there is no additional guidance issued. 2. Eviction by Landlord 14-Day Notice to Pay Rent or Vacate (Residential) - $60 (An attorney is working to update the kit due to new law issued in June, so the kit is temporarily not for sale. Please call or email to be added to a waiting list so we can notify you when the updated kit is ready. Prior kits are no longer valid.)
The kit includes instructions and forms needed to process a basic eviction when the tenant has stopped paying rent or the lease has expired and the tenant won't leave. It was completely rewritten in 2019 due to major changes in the law and is being updated now due to changes made in the 2020 legislative session.

3. Probate Without Will (Intestate) - $30 (updated 7/2020 but the prior version is still valid)
The kit includes forms and instructions to open and close a simple solvent estate in Washington when the person who died did not have a Will. It does not cover administration of the estate, or situations where an heir is a minor or incapacitated person.

5. Garnishment - $30 (updated 5/2020 but the 8/2019 kit is still valid)
The kit includes forms and instructions to process a garnishment after a money judgment has been awarded.

6. Contractor's Bond Claim - $15 (updated 6/2019 but earlier kits are still valid)
The kit includes forms and instructions to start a case against a contractor's bond by a homeowner or supplier/subcontractor when the contract was breached or performance was negligent. It does not cover procedures after the initial filing, and is not intended for electrical contractors.

7. Gun Rights Restoration - $30 (updated 6/2019 but earlier kits are still valid)
The kit includes forms and instructions to request the restoration of gun rights lost due to Washington State convictions.

8. Transfer on Death Deed - $30 (updated 6/2020, but earlier kits are still valid if you use an updated Real Estate Excise Tax Affidavit)
The kit includes forms and instructions to create, record, revoke, or claim under the deed.

9. Starting a Civil Lawsuit in Superior Court - $15 (NEW 9/2020)
The kit includes forms and instructions to start a civil lawsuit in Superior Court against a person. It does not cover procedures after the initial filing and is not intended for District Court.

10. Sealed name change for domestic violence survivors - FREE (updated 4/2018)
This kit is available for download due to the generosity of the King County Law Library. You can change the heading on the forms to indicate Clark County rather than King County.
